✨TO ICE OR NOT TO ICE?✨
.
How many of us have rolled an ankle before, and immediately, without thinking or hesitating, proceeded to ice the injury afterwards? It’s a universal rule to take out the ice packs and start applying it over the area when an injury happens! But what if you were told icing may actually be harmful and may actually interfere with the natural healing process after an acute injury❔ .
.
There are two dichotomies that rehab professionals take with the icing after injury theory. Even in physio school, we have professors with opposing opinions❗

Advocates in favour of the icing theory argue that icing can reduce inflammation and help with the healing process after an acute injury. The application of ice ⬇️ the core temperature of the injured tissue which therefore reduces the rate of cellular metabolism in that area. During an acute injury, the areas surrounding the injured tissue will start dying due to tissue hypoxia (not getting enough oxygen) in a process known as SECONDARY INJURY. Since ice ⬇️ the cellular metabolism, the surrounding tissues need for oxygen decreases and this ⬇️ the risk of secondary injury. .

Proponents against icing argue that the inflammatory process is a natural part of the body’s healing process. Inflammation is the body’s attempt to start removing the damaging stimuli and injured tissue by bringing in chemical and cellular mediators to initiate the healing process. During the stages of healing, the body goes through 3️⃣ different phases, starting with the acute “inflammatory” phase, followed by the proliferative phase, and finally the remodelling phase. If icing prevents the inflammatory process from progressing, then doesn't this delay the body’s ability to advance to further stages in healing❔

Stay tuned for some interesting research against icing! Whether we should or shouldn’t be icing is a hot debate right now, and thus warrants a discussion! Coronal FS T2W MRI of the posterior ankle in a toddler with juvenile idiopathic arthritis (JIA) shows complex anatomy outlined by synovitis, including various ligaments: deep deltoid (blue), posterior inferior tibiotalar (yellow), posterior talofibular (pink), & calcaneofibular (green). The posterior tibial, flexor digitorum longus, & flexor hallucis longus tendons (white, medial to lateral) & peroneal tendons (orange) are also noted. The increased signal of the tibial & fibular epiphyses as well as talus & calcaneus is due to volume averaging of ossification centers & unossified cartilages.
