Median sinirin el bileğinde, transvers karpal ligaman altındaki karpal tünelde basıya uğraması sonucu
gelişen nöropatidir.
KTS'li hastalarda intrakarpal kanal basıncı artmıştır.
En sık görülen bası nöropatisi olup sıklığı genel popülasyonda %1-5, endüstriyel alanda %5-15 olarak bildirilmektedir.
Etyolojisinde mesleki/uğraşsal tekrarlayan travmalar yanında
diyabet,
hipotiroidi,
romatoid artrit,
kronik renal yetmezlik,
amiloidoz gibi sistemik hastalıklar,
obezite,
hamilelik ve
radius distal uç fraktürleri
yer almaktadır.
KLİNİK BULGULARI
-İlk üç parmakta ağrı,
-Uyuşma, -Özellikle geceleri belirgin olan parestezi yakınmaları, -Pozitif Tinel (el bileğinde median sinir üzerine hafifçe vurulması ile eğrı oluşması) ve Phalen (hasta el bileğini
30 saniye boyunca tam fleksiyonda tutup daha sonra serbest bıraktığında uyuşma hisseder)
BELİRTİLERİ
-Elin volar median bölgesinde duyu kaybı, -Tenar atrofi -Başparmak abdüksiyon ve opozisyonunda zayıflıktır.
Kesin Tanı;
elektronöromiyografi ile konur. Tedavide el bileği nötral pozisyonda el bilek istirahat ateline alınır. Elini kullanarak çalışan işçilerde yapılan bir çalışmada 6 ay süreyle geceleri istirahat ateli kullanılmasının KTS'de etkili olduğu ve etkinliğinin bir yıl sürdüğü gösterilmiştir. ❗️El bileğini aşırı fleksiyon, ekstansiyon şeklinde zorlayan hareketlerden, vibrasyon travmasından kaçınılır; el aktiviteleri modifiye edilir. Ultrason, lazer, sürekli düşük seviyeli ışık uygulaması gibi fizik tedavi modaliteleri uygulanır

❌Pain in the Front of your Arm might be referred from the back of your shoulder!

My last post we talked about trigger points. A common trigger point I consistently find is on the back of the shoulder. Infraspinatus, a rotator cuff responsible for shoulder external rotation, has a referred pain pattern to the front of the arm.

When you round your shoulders, internal rotation of the humerus and protraction of the scapula occur. These positions are typically seen in throwing, swinging, sitting posture at a desk/car. In order for your body to maintain these positions, a force couple is required (groups of muscles that act together or in opposition). Therefore to help control the amount of internal rotation, infraspinatus and other external rotators will have to counter that force. Therefore with these repetitive motions or prolonged positions, infraspinatus is loaded in an elongated position and potentially overworked.

To release the trigger point I recommend using a lacrosse ball 🔴. There are a few knots, but the one associated with anterior arm pain is usually located on the upper portion of the muscle closer to the mid-line of the body. See above ☝️
1️⃣Place the ball on infraspinatus and lean on a wall - avoid rolling on bone.
2️⃣Move the ball around (searching for tender spots) while leaning on the wall. Roll on the ball for 30s to 2 minutes.
**Initial dull discomfort when rolling the knot is normal and should subside during the recommended duration**
