OVERVIEW: Shoulder Impingement ("Swimmer's Shoulder") FACTS:
1️⃣ Shoulder pain is the most common musculoskeletal complaint among swimmers.
2️⃣ Impingement syndrome is the most common swimming-related overuse injury in triathletes.
3️⃣ “Swimmer's Shoulder" is a broad term describing a range of painful overuse injuries affecting the shoulder.
4️⃣ Typical Iron-distance triathletes swim 210 +/- 109 minutes per wk, or 8.0 +/- 4.2 miles per wk.❗️
5️⃣ Competitive swimmers may exceed 4000 strokes for one shoulder in a single workout. 💪 ————————————————————————According to Tovin et al., 2006, "Swimmer's Shoulder" typically presents as subacromial impingement involving the rotator cuff tendon, biceps long head tendon, or subacromial bursa. Primary impingement involves compression, or "impingement," of one or more of these soft tissue structures between the acromion and the greater tuberosity. This dysfunction occurs in triathletes often as a result of faulty swim mechanics and/or neuromuscular imbalance(s). 🤔 Mechanical risk factors and contributors in freestyle swimming include: 👇
1️⃣ Hand entry crossing midline.
2️⃣ Thumb-down entry.
3️⃣ Asymmetric trunk rotation.
4️⃣ Unilateral breathing. ————————————————————————One of the most common freestyle swimming technique errors resulting in impingement is inadequate or excessive body roll. When swimming with excess body roll, the arm may cross the midline during the pull through phase. This midline crossing results in increased horizontal adduction of the humerus, putting the shoulder in a position of impingement. Conversely, when swimming with inadequate body roll, the humerus will compensate by moving further into horizontal adduction to allow sufficient propulsion. This, again, places the joint in a position prone to impingement. 👎 While those with a history of impingement can attest that it is not much fun, it is luckily a treatable condition and responds well to conservative interventions. Stay posted for more specific interventions and exercises to prevent and/or treat Shoulder Impingment. 👍
