#backtothefuture #fromlasttofirst #Preoperative #photograph of a #traumatic #cataract and #pupillary #block #glaucoma patient who had previously undergone #ALPI and #laser #iridotomy. Unfortunately the zonule support was less than 90 #degrees. So I made #ICCE. Then i have done #scleralfixation #IOL and #siepser #suture #pupilloplasty

#preALPI AND #postALPI #biomicroscopy of #traumaticeyeinjury . #preperation for #iridotomy and #LENS #ekstraction #BECAUSE OF #ZONULAR #dializis #pupillary #BLOCK #ANGLE #clouser #GLAUCOMA #glaucomaawareness #Argon #laser #peripheral #iridoplasty (#ALPI) is a simple and effective means of opening an appositionally closed angle in situations in which laser #iridotomy either cannot be performed or does not eliminate appositional closure because mechanisms other than pupillary block are present. It consists of placing contraction burns (long duration, low power, and large spot size) in the iris periphery to contract the stroma at the site of the burn, physically pulling open the angle. ALPI is useful in reversing an attack of #acute angle-closure (AAC), either as an initial measure or when #antiglaucoma medications fail. The diagnostic indications for #ALPI require the ability to differentiate subtle gonioscopic findings. The examiner must be facile with darkroom #indentation #gonioscopy and with the #anatomic #causes of angle-closure and the means of #diagnosing these clinically.

Persistent #pupillary membrane (PPM) is a condition of the eye involving remnants of a fetal membrane that persist as strands of tissue crossing the pupil.

The pupillary membrane in mammals exists in the fetus as a source of #blood supply for the #lens. It normally #atrophies from the time of birth to the age of four to eight weeks.
PPM occurs when this atrophy is incomplete. It generally does not cause any symptoms but can reduce VA if severe.

The strands can connect to the cornea or lens, but most commonly to other parts of the iris. Attachment to the #cornea can cause small corneal opacities, while attachment to the lens can cause small #cataracts.
Using topical atropine to #dilate the pupil may help break down PPMs and only in #extreme cases will #surgery be required. .
.
PS: Apart from PPM what else can you #notice about this #eye?? 👀
