Truth.

When I’m asked how I could walk away from running & not lose my mind like every single runner on the planet believes to their core- you won’t. Running served me well until it didn’t. Period. I moved on to other means of torture that I love. Do I miss it? Well surgery scheduled next week —again, with the strong possibility for a total knee replacement early 2018. So my answer, no I don’t miss it. Keep your eyes forward, even if the tears are falling & know you are strong enough for whatever is thrown at you.

Meniscus tears are one of the most common sports injuries patients face today. The meniscus acts like a shock absorber in the knee joint. There are two meniscus in each knee. Each meniscus plays an important role in knee stability and protects the cartilage in our knees. Often times a meniscus cannot be repaired and the torn tissue must be removed because of the symptoms it causes. This procedure is known as a partial meniscectomy. However, when removing meniscus tissue the joint has less protection and faces more contact pressure and force likely accelerating arthritis. It is in the best interest of the patient to only remove damaged meniscus if it is causing true mechanical symptoms and if it absolutely cannot be repaired. In the upper left there is a bucket handle medial (inner side) meniscus tear. The schematic drawing in the lower left hand corner demonstrates the same tear pattern. The upper right demonstrates the tear after it has been repaired. And the lower right demonstrates a normal intact lateral (outer portion) meniscus of the same knee for comparison. You can imagine that if we removed that torn tissue in the upper left instead of repairing it, the patient would have been left with very little shock absorber (meniscus). Many patients after repairs or a partial meniscectomy have quadriceps weakness that is important to address with a therapist to optimize their outcome!

The knee is one the joints where the diagnosis should be made come the end of the subjective assessment for the majority of pathologies.

Let's look at 3 injuries and how the symptoms present.
.

Injuries covered:
.

1. ACL rupture- in these cases the patient will always present with a specific incident of injury. The client usually reports a twisting mechanism of the knee or some sort of direct blow. The pain is huge immediately but usually lasts a few minutes. The knee is then relatively pain free. The person will be unable to continue activity. The knee may swell, however this isn't always the case. The knee becomes unstable as a result of the rupture. On examination ROM is usually full. Quads bulk maybe reduced, ACL maybe positive (anterior draw test), but this is not always the case.

2. Meniscal injury- these injuries can occur in conjunction with an ACL injury. Therefore. The mechanism of injury is similar. Patients normally present with an effusion on the knee. If seen acute then the patient may not be able to achieve full extensions pain may well be felt at the end of range of motion in both directions. Meniscal testing can be positive or negative (McMurrays test). Palpating of the joint like may well be tender.

3. Chondral defect- in these cases both the cartilage and the bone underneath tears off the joint causing a plug like appearance. This is due to a traumatic incident. Again there is usually a swelling associated with this injury. Pain is felt within the knee and usually continues to pain the person if the management isn't correct.
