mrs_angemi Yes! I'm glad most of you recognize this! Twin to twin transfusion syndrome (TTTS) is a disorder
of the placenta and affects identical twin pregnancies who share a common placenta (monochorionic). The shared placenta contains an abnormal pattern of blood vessels, which connect the umbilical cords and circulations of the twins. The blood vessel circulation may be shared unequally causing one twin (recipient) to grow too much, and the other twin (donor) to grow too slow. The transfusion of blood causes the donor twin to have decreased blood volume. This in turn leads to slower than normal growth and poor urinary output causing little to no amniotic fluid (oligohydramnios). The recipient twin becomes overloaded with blood. This excess blood puts a strain on this baby’s heart to the point that it may develop heart failure, and also causes this baby to have too much amniotic fluid (polyhydramnios). TTTS can be chronic or acute. Chronic TTTS occurs early in pregnancy (2nd trimester). These cases are the most serious because the twins will have a longer time during their development in the womb to be affected by the TTTS abnormalities. Acute TTTS describes those cases that occur suddenly, towards the end of the pregnancy. These twins may have a better chance to survive based on their gestational age, but may have a greater chance of surviving with health issues. In this particular case, there is slight swelling and skin slippage due to the babies dying in utero. In children's hospitals placentas that have suspected TTTS are treated special. The umbilical cords of each baby are injected with a different color dye and then the vessels on the disc are examined looking to see if they connect or not. If the 2 dyes connect this means the twins definitely has TTTS.
